Coronavirus Pre-Visit Self Assessment
Before your visit with us, it is required that you review a set of health related questions as a COVID screening. Please review these questions at least 48 hours prior to your appointment. Depending on your answers, you may need call our office at (206) 524-1000 to reschedule your appointment. We’re doing our best to keep everyone safe, and look forward to seeing you soon.
- Do you or have you had any flu-like symptoms such as fever, chills, muscle aches or headache in the last 14 days?
- Any other flu-like symptoms such as gastrointestinal upset, diarrhea or nausea in the past 14 days?
- Do you have shortness of breath or a cough (beyond what you consider a normal allergy related symptom)
- Have you experienced a recent loss of taste or smell?
If “Yes”, please contact our office today to reschedule your appointment at least 14 days from the date of your last symptoms.
- Have you had close contact to an individual diagnosed with COVID-19 infection in the past 14 days without proper PPE?
If “Yes”, please contact our office today to reschedule your appointment at least 14 days since your exposure date.
- Have you tested positive for Covid-19?
If “Yes”, please contact our office today to reschedule your appointment at least 14 days from date of last symptom.
- Are you awaiting results of a lab test?
If “Yes”, please contact our office today to reschedule your appointment at least 7 days from date of a negative test result.
- Do you have heart disease, lung disease, kidney disease, diabetes, autoimmune disease or undergoing chemo or radiation therapy?
If “Yes”, consult your MD or DDS regarding urgency of care.
